Hi. Is___ available? Hi ____. My name is _____ and I'm a Navigator with ASPIN. I/One of my co-workers helped you/your family with Medicaid/Marketplace health insurance. I’m reaching out because I have a few reminders and some questions regarding your health coverage. Although you can get healthcare in many different places, including the emergency department, it’s best for you to get routine care and recommended preventive services from a primary care provider. Having a primary care provider can save you time and money.

I. Select Primary Care Provider

Insurance companies have contracted with specific facilities, providers, and suppliers to provide healthcare services. If you pick an "in-network" provider, your insurance will cover more of the costs. If you pick an "out-of-network" provider, you will have to pay most or all of the costs out of your own pocket. You can check to see if your provider is "in-network" by contacting your insurance company.

II. Preventive Screening/Services Reminder – Preventive screenings and services are free. Routine checkups and screenings can help you avoid serious health problems and/or obtain early diagnosis and treatment, allowing you and your doctor to work as a team to manage your overall health. Some examples of preventive screenings include blood pressure, depression, diabetes, immunizations, breast cancer and tobacco use.

At www.healthfinder.gov you can type in your gender and age to receive a list of recommendations to stay healthy.
